Scope Of Work

SCOPE OF WORK PHASE I OF STRUCTURAL 

SLAB REPAIR, RESTORATION AND WATERPROOFING FOR

VILLA ISABELLA CONDOMINIUM

NOVEMBER 2016

PROJECT DESCRIPTION

Villa Isabella Condominium is a 29 unit apartment building located at 322 Madeira Avenue in Coral Gables, Florida.

The purpose of this project is to repair and restore the ground floor slab and remove-replace the existing waterproofing membrane over the parking garage slab and planters. The damage to the slab and existing waterproofing membrane to be best of knowledge and  professional opinion is the cause of the water intrusion thru-out the slab.

It was revealed during a photographed investigation performed in a section of the rear center planter on April 28, 2016 that the product used for waterproofing visually appeared to be a felt membrane attached to the slab with asphalt.

In addition, a small area in the front northeast planter was also investigated and photographed. The location of this small area is under a balcony within the planter. A small section of the wall felt flashing was lifted it revealed a waterproofing product applied over the slab. The applied waterproofing product applied in this area as well as in the large planter adjacent to the balcony visually appeared not be functioning as designed or as applied.

Analyzing the conditions and water intrusion issues of the structural slab over the parking garage, the proposed  process to carry out this project is for it to be divided into three (3) phases. Phase I. Design and City of Coral Gables approval of the following: new landscaping, demolition of existing landscaping and waterproofing, structural design of repairs, restoration, and waterproofing of the structural slab and design of restoration of backfill, top-soil and planting of new ground cover, shrubs, plants and trees. Phase II. Structural slab repairs, restoration and waterproofing. Phase III. Landscape restoration.

Phase l -    Landscape permit document preparation, permit document submission to City of Coral Gables, 

                  acquisition of landscape demolition, soil, backfill and membrane removal permit. Removal and disposal

                  of existing trees, stumps, root systems, shrubs, top-soil, back-fill and membrane.

                  Structural Engineer’s inspection, preparation of permit documents and specifications for permit 

                  submission to City of Coral Gables.  Acquisition of repair, restoration and waterproofing permit by 

                  DESIGN-BUILDER’S specialty structural repair and waterproofing application subcontractor.

 

Phase II -  Structural repairs, restoration and waterproofing application to damaged slab as per Structural Engineers    

                 specifications and approved permit documents by the City of Coral Gables. Work in Phase II shall  be 

                  performed by the DESIGN-BUILDER’S specialty sub-contractor, required subcontractors and other 

                 required labor and supervision. A Change Order for the additional cost of this work shall be added to the

                 DESIGN-BUILD CONTRACT.

 

Phase III- Restoration of planters, planting of shrubs, ground cover, plants and trees as per approved Landscape 

                Plans by the City of Coral Gables. Restoration of exterior walkways (if required) by Engineer’s 

                specifications and report. A Change Order for the additional cost of this work shall be added to the

                DESIGN-BUILD CONTRACT.

 

Scope of Work for Phase I and the Lump Sum Contract Price for the outlined work is herein included. Submission of the Scope of Work for Phase II and its cost cannot be determined until completion of the Engineer’s specification and corrections of City of Coral Gables comments, if any. Preparation and completion of Engineer’s specifications cannot occur until completion of the work in Phase I.  Upon completion and construction permit document approval by the City of Coral Gables of the Structural Engineers report and design for repairs, restoration and waterproofing of the ground floor structural slab the cost for Phase II shall be determined by the DESIGN-BUILDER, cost of this work shall be added  to 

DESIGN-BUILD CONTRACT as a Change Order. A preliminary Cost Budget for Phase III as per the approved City of Coral Gables Landscape permit documents will be prepared within 25 business days of landscape permit document approval by  the DESIGN-BUILDER. Upon completion of the work of Phase II a final cost for Phase III shall be determined by the DESIGN-BUILDER and a Change Order for the additional cost of Phase III shall be issued and the additional cost added to the DESIGN-BUILD CONTRACT.

SCOPE OF WORK - PHASE I 

1.       Contracting, coordinating, and managing of a Licensed State of Florida Landscape Architect for the   

          preparation of permit contract documents to be submitted to the City of Coral Gables for the 

          acquisition of a permit for the removal and replacement of the landscaping.   

2.       Submittal, presentation and approval of new landscaping plan to City of Coral Gables Board of 

          Architects, if required, or to the City of Coral Gables City Architect.

3.       Preparation and submission of required documents to the City of Coral Gables for the acquisition    

          of General Contractor’s Master and Landscape permits for the removal of all existing trees,

          palms, shrubs and soil and membrane.

4.       Removal and disposal of all existing trees, palms, shrubs, tree stumps, root systems, soil, sprinkler system 

          and pump and deleterious material from planters.

5.       Removal and disposal of membrane and asphalt on slab and planters, broom clean to expose slab.

          Enabling Structural Engineer to inspect the visual damage.

6.       Installation of temporary construction safety fences.

7.       Preparation and approval by City of Coral Gables of construction staging plan.

8.      Contracting, coordinating, and managing a Licensed State of Florida Professional Engineering firm  

         for inspection, analysis, design, recommendations, preparation of permit contract documents and 

         specifications required to repair, restore and apply waterproofing to the damaged slab.

9.      General Contractor to obtain all licenses, certificates, and insurance required by law.

10.    General Contractor and Subcontractors to have and maintain a Liability Insurance Policy

         in the amount of $1,000,000 naming Villa Isabella Condominium Association as an additional

         insured. General Contractor and Subcontractors to have and maintain a Workmen’s

         Compensation insurance policy as required by State of Florida Statures.

11.    Estimated time for the completion of Phase I is 80 business days after execution of the 

         DESIGN-BUILD CONTRACT between (OWNER) Villa Isabella Condominium Association and 

         (DESIGN-BUILDER) Sunburst Construction Management Company Inc. Construction permit 

         document preparation by Landscape Architect for Phase I and approval by the City of Coral Gables is 

         estimated at 50 business days. Performance of demolition work of Phase I is estimated at 30 

         business days. ( See Exclusion 8E)

EXCLUSIONS FROM SCOPE OF WORK - PHASE I

 

1E.      Payment of any and all type of permits, impact fees, taxes and assessments levied by The City of  

           Gables, Miami-Dade County, State of Florida, United States Federal or Environmental Agencies as

           as it pertains to this project     

2E.      Removal and disposal of brick pavers and stamped concrete walkways.

3E.     Contracting and payment of Inspectors, owners representative and certifications not specifically   

           outlined in the Scope of Work of Phase I.

4E.     Work not specifically outlined and detailed in contract documents as part of Phase I. Should

          OWNER request additional work not specifically outlined in Phase I, this requested new work  is

          to be considered additional work and shall be performed at an additional cost. Written Change 

          Order authorization by the OWNER is to be issued, approved, and a pre-requisite to the 

          commencement of any additional work.

5E.     Payment of any kind required for the relocation, alteration, disconnection, connection, and any               

          and all fees required to be paid to Florida Power and Light Company, Miami Dade Water and

          Sewer Authority, City of Coral Gables, ATT, any television cable company and any and all type

          of service utility company serving Villa Isabella Condominium.

6E.    Liability for damages and protection of vehicles and any kind of property located in the underground

         parking garage while carrying out the construction demolition, repair, restoration, or any other kind 

         of construction activity that occurs while carrying out this project. A parking and protection staging 

         plan can be arranged and agreed upon by the OWNER and the CONTRACTOR.

7E.    Any kind of work, permit cost, fees, or services required unless otherwise specified for Phase II or 

         Phase III of the Project Structural Slab Repair, Restoration and Waterproofing for Villa Isabella 

         Condominium located at 322 Madeira Avenue Coral Gables, Florida 33134. 

8E.   Completion schedules for Phase I, Phase II and Phase III are estimates based on our past 

        experience and to the best of our professional knowledge and belief. Weather delays, acts of 

        God, conditions beyond the control of the DESIGN-BUILDER and unforeseen hidden conditions 

        shall affect the cost and completion schedule of the DESIGN-BUILD CONTRACT.

9E.   Repair and replacement of hidden damages caused by the growth of the existing trees root systems, 

        by the removal of the existing trees root system, by the grinding, removal of the tree’s stumps and root 

        systems to the drainage pipe or drain-field which may run between the two (2) catch basins located in the east 

        and west ends of the property said drainage pipe and drain-field is located approximately four feet south 

        of the rear planter’s retaining wall parallel to the southern property line.
